Probably just a really strong pickled ginger thing, meant to cut through the richness of the food. Some places go hard on the spice and vinegar in their pickles, especially in @Bangkok. People might just not be used to that level of intensity, or maybe it's a regional variation that's not as popular outside a smaller area. I wouldn't worry about missing some deep cultural significance. It's probably just… intensely pickled ginger.
