Taxi scam or am I missing something?
I am afraid at… When ordering a grab and a taxi shows up, half of them start driving but it doesn't show on the app, so I ask them to notify they've picked me up, and get out of the car if they don't.
I am afraid at the end of the ride that they will request their own fare since the ride is not launched in the app and the taxi-meter not on. But on the other hand I suppose they can get penalty if they start driving away from the pick-up point without notifying that they picked me up in the app.

You're right to be wary. What's happening is some drivers accept Grab jobs then try to take you off-app to avoid paying Grab's commission, which is around 20-30%. If they start driving without the trip showing in the app, they're essentially running their own private meter.
If they pull that, just ask them to stop and get out. They'll lose face and move on to the next person. I'd stick with the meter if you're hailing a taxi on the street, but for Grab always make sure the app shows the trip has started before you let them drive off. The penalty for drivers who do this is real, but Grab's enforcement is spotty so you're better off just refusing the ride.
This is a common enough issue that I've just started checking the driver's phone screen before getting in. If they can't show me the Grab interface with my ride details, I don't get in. Saves the awkward conversation about starting the trip. Drivers near Nana and Asok seem to try this more often, probably because they know tourists are in a hurry.
You've got the right instinct. Some drivers use a second phone or just turn off their data hoping you won't notice and they can collect both the Grab fare and whatever cash they demand. I've had it happen twice near Ekkamai and both times I just opened the door at the next red light and walked away. The driver can't report you to Grab because they never started the trip, so you're not on the hook for anything.
