Long story short, I've been dealing with a mystery disease for several years now and I seem to be getting worse. I'm from the states and have done some basic testing but have never really done anything specific to cancer screenings.

I'm not back home for a few months and have felt this sudden urge to get some testing done as soon as possible, so I thought I would try here.

Can anyone recommend the best options for me here? Cost isn't really a concern, I just would like to visit the best doctor/facilities to discuss options and tests I could do. Someone who specializes in this field.

Additionally, if there are any really good doctors here that could potentially help with a diagnosis, that would be an additional goal (but my main concern for now is cancer screening). Doctors back home have diagnosed me with CFS/ME but I would like to see if there isn't something more going on.

fwiw I'd skip the big hospital factory line and go see Dr. Somsak at the Thai Red Cross AIDS Research Centre clinic. yeah the name is misleading, they do way more than HIV stuff now, their tropical medicine and cancer screening unit is actually legit and cheap since it's a nonprofit. he's a hematologist who used to head the cancer ward at Chula and will run the full panel of tumor markers plus flow cytometry for like 3000 baht total, no appointment needed just show up early

for the mystery illness angle, try the Immunology department at Siriraj Hospital. Dr. Mana there is old school and doesn't do the CFS brush-off, he'll run you through the autoimmune encephalitis panel and the whole paraneoplastic workup. it's a public hospital so expect a wait but the level of investigation is way deeper than any private place I've been to. bring all your old records translated if possible, he actually reads them

one thing nobody mentioned is that most screening packages miss the rare stuff like neuroendocrine tumors or sarcomas because they focus on the big five cancers. ask whichever doctor you see to add a chromogranin A test and a urine catecholamine panel, it's cheap and catches what the MRI skips. I had a friend with similar symptoms who got diagnosed with a pheochromocytoma that way after years of being told it was anxiety

If you want a second set of eyes on the whole mystery illness angle alongside the cancer screening, try Dr. Jiraporn at the Preventive Medicine Clinic at MedPark Hospital. She practices what they call "executive health" but she's actually thorough, not just a checklist. She spent years at Mayo Clinic and will sit with you for 45 minutes going over your history, something most doctors here won't do.

For the scans themselves, avoid the tourist packages and go straight to the Nuclear Medicine department at King Chulalongkorn Memorial Hospital. Their PET/CT is about half the price of the private hospitals, around 25,000 baht, and the radiologists read for both Chula and Bumrungrad anyway. You'll need a referral from a doctor to book it directly.

One thing nobody talks about is that Bangkok Hospital's lab has a partnership with Quest Diagnostics in the US for sending out rare biomarker panels. If Dr. Jiraporn isn't sure what to test, she can order stuff like the cytokine array or the mitoswitch panel that most local labs don't run. You'll wait a week for results but it's the only place in the city doing that level of obscure testing.

ngl if you're dealing with a mystery illness that's getting worse, i'd go straight to Dr. Suthep at BNH Hospital's internal medicine department. he's an immunologist who actually listens and won't just slap a CFS label on you. his full workup runs about 15k baht for the consult and initial labs, but he'll order things like the NK cell function test and the viral reactivation panel that most doctors skip entirely. i've seen him catch a chronic HHV-6 infection in someone who'd been told it was anxiety for years.

for the cancer screening specifically, the real hidden gem is the Radiology Center at Thonburi Hospital. they have a 3T MRI that most places don't bother with and their whole-body diffusion sequence catches stuff at 3mm instead of the standard 5mm threshold. the radiologist there, Dr. Anan, used to teach at Ramathibodi and will actually call you personally if he sees anything suspicious. full body scan runs about 35k baht which is less than half of what Bumrungrad charges for the same machine.

one thing to keep in mind, if you're doing the PET/CT at any of the public hospitals, get the one with the time-of-flight detector. Chula has it but their older machine doesn't, and the difference in image quality is night and day for small lesions. just ask the booking office specifically for the TOF scanner

yeah Krua Dok Mai Kao is solid but if you're up for a detour, try Som Tum Nua near Siam Square. their som tam pu pla ra is insane if you like the fermented fish sauce kick. way more pungent than the On Nut spot, just less grilled chicken options
