For a year you'll want the work or student route, not the 90 day tourist extension, and bring photocopies of everything.

Bring two passport photos, they always ask for more.

The real bottleneck is the bank statement stamp and the lease notarized at the local shahr aqari office, so sort those first.

honestly the citadel area is mostly government hospitals, not great for a foreigner with insurance. i'd look at a private clinic in Nasr City or Heliopolis instead, 20 min by Uber, and most take international plans. cash consult runs 500 to 800 EGP there.
