I'd skip Nai Kao entirely and head to a place like Rong Si or even a simple boat noodle stall on Sukhumvit Soi 38. The trick is to tell them "pet mak mak" (very very spicy) and watch their face for a smirk before they nod. At Rong Si, their tom yum noodles come with a broth that actually burns in a good way, and they won't judge you for sweating through it. If you want real heat, avoid places with English-heavy menus or influencers in the photos.

ngl you prob just got the tourist version. Nai Kao has two kitchens basically - one for locals one for farang. If you sit near the back by the big wok you'll hear them shouting orders in thai that are way different than what comes out front. Next time just say "pet thai" and dont smile when you say it. Or go to Jok Prince down the street, their rice soup looks innocent but they have a chili set thatll make you rethink your life choices

The real trick isn't a secret menu item, it's knowing the right phrase and the right tone. Say "pet thai" or "pet mak mak" firmly when you order, and if you're sitting at the counter, watch them prep it. Some cooks will test you by asking again after you say it, just nod and repeat yourself.

For proper heat without the game, walk over to Kuay Jab Mr. Jo in the same area. Their crispy pork rolls come in a peppery broth that's already got a kick, and they have a jar of pickled chilies on every table that'll actually make your ears ring. No translation needed, just grab a spoonful and drop it in.

yeah Krua Dok Mai Kao is solid but if you're up for a detour, try Som Tum Nua near Siam Square. their som tam pu pla ra is insane if you like the fermented fish sauce kick. way more pungent than the On Nut spot, just less grilled chicken options
