counter 3 is the one for births actually, counter 4 handles deaths. go on a tuesday or thursday, mon is chaos with the weekend backlog. bring exact change for the form fee, they never have change
tuesdays are definitely better, but check the ward's holiday list first since some are closed for local staff meetings. also they only take cash for the late fee if you're past 21 days, so keep a couple hundred on u.

At the ward counter they usually want the parents' marriage certificate plus an attested copy of your wife's Aadhaar, not just yours. The self declaration doesn't need notarising, a plain signature works, but write it on plain paper with the date.

Agree on the declaration. Also carry your daughter's immunization card if you have one, they sometimes ask for it as proof of address. And ask for the acknowledgment slip when you submit, they won't offer it unless you do.

nsdl portal lets u apply online & pick ur nearest centre for biometrics after landing. for address just use ur company's Mumbai office address with the rent agreement or utility bill in the employer's name, they accept that for new joiners
the utility bill trick only works if it's recent, like within 2 months, so ask ur HR for a fresh one not an old copy
