Bring NSO or PSA birth cert and marriage cert copies, they always catch people out, plus bank statements from teh last 6 months.

Bank statements need the embassy's six-month stamp

I'd check the embassy site directly for the current long stay visa list, rules shift year to year.

Which hospitals or clinics near Makati would you recommend for a walk in consultation, and roughly what does it cost if I'm paying cash? Also, do most places take international travel insurance or should I expect to pay upfront and claim later?

Makati Med, derm clinic 3rd floor, cash first.

Second this, bring pesos cash, insurance claim later.

arrow_drop_up 1 arrow_drop_down

cash upfront is right but get an itemized receipt & a doctor's letter with the diagnosis, insurers ask for both. some clinics take card now tho, ask before u queue

arrow_drop_up 1 arrow_drop_down

true, though the atms around ayala can be hit or miss on weekends so withdraw the day before. also ask for the receipt in ur passport name, not a nickname, or the claim gets bounced

arrow_drop_up 3 arrow_drop_down

yep 3rd floor derm is right, just book the earliest slot since walk-ins queue past an hour by 9am

arrow_drop_up 2 arrow_drop_down

philhealth accredited too if u want the cheaper rate
