depends on context tbh. if someone says "im going to tokyo" they usually mean the 23 wards, like shinjuku or shibuya. but officially "tokyo" is the whole prefecture, which includes places like hachioji and tachikawa out west. locals will say "tokyo" for the city center and "tokyo-to" when talking about the whole thing, or just name the specific ward

The western suburbs like Mitaka or Kichijoji are technically Tokyo but people usually call them by name.
