fwiw the national hospital organization tokyo medical center in meguro has a solid respiratory department. it's a big public hospital so they're used to handling chronic conditions like asthma and the doctors there are pretty thorough with treatment plans. your wife should ask about 呼吸器内科 and mention you need a long-term management setup, not just a one-off prescription. also if you're near setagaya, the setagaya asthma clinic in soshigaya is a small specialized place that's known for being patient with foreigners even if the english is basic. just have her confirm the paperwork stuff upfront because japanese hospitals love their forms

if you're in central tokyo, st. luke's international hospital in chuo-ku has a respiratory medicine department that's used to foreign patients. they have an english appointment line and the doctors are generally up to date on global asthma guidelines. your wife can still help with the initial call but you won't be totally lost once you're there.

for something more neighborhood-level, the hiroo clinic near hiroo station has a few internists who handle asthma well and the staff is comfortable with english. it's a smaller place so you get more time with the doctor, but they're not a full hospital if you ever need emergency care. worth having both options lined up.

Dr. Saito there is known for being meticulous with asthma action plans and peak flow monitoring. your wife will need to handle most of the talking since english is limited but they're used to foreign patients. i'd also recommend getting a referral from a local clinic first, it's cheaper and faster than going straight to a big hospital. the cedar pollen season is brutal so ask about getting a prescription for antihistamines and nasal spray early
