honestly i'd try the tokyo medical center in meguro, they have a travel clinic that does paid covid shots with no appointment needed. think it was like 15k for moderna last time i checked. theyre used to random walk-ins asking for stuff like yellow fever or hep a so they wont bat an eye at a covid booster
also worth hitting up some of the bigger chain clinics like soukai or midori, they sometimes have a separate "vaccination only" window where you can pay cash without needing a regular exam. the receptionists at those places are less likely to give u the runaround compared to tiny clinics where the doctor has to personally approve everything
