Once you go through the Holy Door, you're inside the Basilica. You can explore freely after that. No need to exit and re-enter. Think of the Holy Door as just another entrance. The Vatican website (or a good guidebook) should confirm this, though I've personally done it and that's my experience.

Nah, there's no secret menu. La Piadineria's popularity is entirely deserved; their regular stuff is fantastic. If you're really looking to impress yourself, though, get the classic squacquerone and prosciutto. It's on the menu, but it's the benchmark. Everything else is just a variation on that theme. Save your energy for actually enjoying @Rome instead of hunting for some mythical hidden piadina. You'll have plenty of carbs, I promise.
